ABSTRACT

This study was designed to investigate the effects of icariin on bone metabolism in osteoprotic mice induced by iron overload, the model of iron overload mice was established by intraperitoneal injection of iron dextran (100 mg·kg-1). Sixty 2-month-old C57/BL6 male mice were randomly divided into six groups, including normal control group, model group, N-acetyl-L-cysteine (NAC)-treated group, icariin (50, 100 and 200 mg·kg-1)-treated group. Except for the mice in control group, the mice were intraperitoneal injected weekly with iron dextran (100 mg·kg-1) to establish the model of iron overload mice. The NAC and icariin were suspended in 0.5% CMC-Na solution, and administered orally for six times one week according to body weight. The mice in normal group and model group were given the same volume of 0.5% CMC-Na solution. Three months later, the organs, serum and femurs of mice were collected. Serum biochemical parameters were detected with an ELISA kit, the distal femur bone density and trabecular bone microstructure were analyzed by Micro-CT, and the mechanical properties of femur were measured by universal mechanical analyzer. Compared with the normal control group, iron overload decreased the bone mineral density and deteriorated the micro-architecture structure and bone mechanical properties in femur of mice, increased the level of iron, phosphorus and activity of tartrate resistant acid phosphatase-5b (TRACP-5b), reduced the level of osteocalcin (OCN) in serum, decreased the activity of superoxide dismutase (SOD) of liver tissues, increased the content of malondialdehyde (MDA) of liver tissues. Icariin increased bone mineral density, improved the micro-architecture and mechanical properties of bone tissue, reduced the levels of iron and phosphorus, decreased the activity of TRACP-5b and enhanced the levels of OCN in serum, and also decreased the activity of MDA in liver tissue of iron overload mice. These results suggest that icariin is able to reduce bone loss and improve bone microstructure and mechanical properties in iron overload mice through regulation of bone metabolism via anti-oxidation.

ABSTRACT

Objective To investigate the effects of repeated low dose intravenous infusion of low molecular weight iron dextran and iron sucrose on oxidative stress in chronic renal failure (CRF) rats. Methods CRF model was established by 5/6 subtotal nephrectomy (5/6 Nx). Four weeks after removing the right kidney, successful rats were randomly divided into low molecular weight iron dextran group, sucrose iron group and CRF control group. The sham group was established simultaneously. The dose of iron administrated in each rat was similar in iron dextran group and sucrose iron group. There were 6 rats in each group. Animals were observed for 6weeks, then the blood, urine and renal tissue samples were collected, and indexes of renal function, anemia, iron status and oxidative stress were investigated. Results The hemoglobulin (Hb) level in iron groups was significantly higher as compared to control group (P<0.05) but was not significantly different between two iron groups. The levels of serum iron, ferritin and saturation rate of transferring (TS) were obviously lower in control group as compared to sham group (P<0.05).Levels of above 3 indexes were significantly higher in two iron groups as compared to control group (P<0.05), but were not significantly different between two iron groups. Concentration of plasma advanced oxidation protein products (AOPP) was obviously higher in two iron groups than that in control group [(127.84±21.19) μmol/L, (134.21±29.38) μmol/L vs (81.83±19.93) μmol/L, P<0.05]. Plasma malonaldehyde (MDA) was significantly higher in iron sucrose group than that in iron dextran group [(6.06±0.73) nmol/L vs (4.99i0.80) nmol/L, P<0.05]. Serum levels of superoxide dismutase (SOD) and total anti-oxidant capacity (TAOC) had no significant differences among three CRF groups. Concentration of plasma glutathione peroxidase (GSH-Px) was significantly decreased in three CRF groups as compared to sham group (P<0.05), while plasma GSH-Px was significantly lower in sucrose iron group than that in iron dextran group and control group [(2123.11±74.78)nmol ·ml-1 ·min-1 vs (2352.84±163.90) nmol· ml-1 ·min-1, (2310.23±125.99) nmol ·ml-1 ·min-1, P<0.05]. Conclusions Injection of intravenous iron can partially improve the anemia and the iron status indexes in 5/6 Nx CRF rats. Repeated low dose intravenous infusion of iron dextran and iron sucrose can aggravate the oxidative stress state in CRF rats, and the iron sucrose is worst.

ABSTRACT

Objective To determine the efficacy and safety of intravenous iron sucrose and oral iron dextran in renal anemia patients. Methods Sixty renal anemia patients were randomized into two groups:intravenous iron group(IV group, 100 mg iron sucrose twice a week) and oral iron group(oral group, 150 mg iron dextran every day). The changes in hemoglobin (Hb),hematocrit (Hct),serum ferritin (SF),and transferrin saturation (TSAT) were assessed on week 4,8, and 12 after treatment. The maintenance dosages of erythropoietin (EPO) after reaching treatment target and the adverse events were also observed. Results Hb increased after treatment in IV group [(79.87 ± 13.19) g/L to (106.11 ± 12.38) g/L] and oral group [(85.41 ± 11.49) g/L to (94.68 ± 10.06) g/L],and compared with that before treatment, there was significant difference in each group(P< 0.01 ). Also significant difference showed in Hb levels after treatment between two groups(P< 0.01 ). SF and TSAT levels were higher after treatment in IV group than those in oral group (P < 0.01 ). The adverse event was lower in IV group (0) than that in oral group (16.7%,5/30).Conclusions Oral iron dextran which has more adverse events and poor efficacy in curing anemia, can not correct anemia ideally. Meanwhile intravenous iron sucrose has positive effects on renal anemia with low adverse events, and it can be used as a long term iron supplementation in hemodialysis patients.

ABSTRACT

OBJECTIVE:To establish the ultraviolet visible spectrophotometry(UV-VIS)for the determination of Fe content in iron dextran preparations.METHODS:Under the acidic condition,Fe~(3+)was dissociated from iron dextran solution by heating, then Fe~(3+)was deoxidized into Fe~(2+)by hydroxylamine hydrochloride, and red Fe~(2+)-orthophenanthroline complex was formed through combination with o-phenanthroline.The content of the Fe~(2+)-orthophenanthroline complex was determined by UV-VIS at a wavelength of 510 nm.RESULTS:The linear range of Fe~(2+)was 0.5~4.0?g?mL~(-1)(r=0.999 9). The average recoveries of two preparations(tablets and injection)of iron dextran were 99.8%(RSD=0.52%)and 99.7% (RSD=0.44%),respectively.CONCLUSION:The UV-VIS method is simple, accurate in results and applicable for the determination of iron dextran preparations.

ABSTRACT

Twenty-eight patients received intravenous iron-dextran (TDI) for correction of iron deficiency anemia. One experienced a systemic reaction but recovered without untoward effects, another developed uterine contraction and both of this case were not included in this studiesThe rise in hemoglobin and hematocrit were noted after two weeks and was rapid over a period of six weeks. No delayed reactions were seen in all the casesWe believe that, although not without danger, the use of Iron-Dextran Infusion (TDI) by the method and techniques described should find a useful place in correction of severe iron deficiency anemia.
